  • If you file Form I-485 online, you must open the sealed envelope from the civil surgeon and upload the completed Form I-693 with your application package. You must keep the original Form I-693 and the envelope from the civil surgeon until we make a final decision on your Form I-485. We may review the original Form I-693 and envelope during an interview or ask you to submit the original Form I-693 and envelope as evidence to support your application. We we you an additional copy so that you can keep the original sealed for your interview
  • ALERT: Effective January 22, 2025, USCIS waives any and all requirements for COVID-19 vaccination. USCIS will not deny any adjustment of status application based on the applicant’s failure to present documentation that they received the COVID-19 vaccination.
  • ALERT: Effective Dec. 2, 2024, if you are required to submit Form I-693, or a partial Form I-693 (such as the Vaccination Record), you must submit it with your Form I-485. Otherwise, USCIS may reject your Form I-485. 
  • ALERT: Polio Vaccine is now required for most patients if you don't have records of vaccinations.
  • ALERT: Effective April 4, 2024, any Form I-693 that was properly completed and signed by a civil surgeon on or after Nov. 1, 2023, does not expire, and can be used indefinitely as evidence to show that the applicant is not inadmissible on health-related grounds. Why is the form I-693 required for immigration purposes?


The Immigration Medical Exam is required for filing form I-485 (Application to Register Permanent Residence or Adjust Status. Once you have your "Green Card," you can become a citizen in 3 - 5 years, depending on your case.


How much does the immigration medical exam cost in Orlando?

Our Immigration Medical Exam pricing varies by age because USCIS requires different laboratory tests for different age groups. All prices include the medical exam and the routine laboratory tests required for your age group. Required vaccines, if needed, are not included.

The exam and paperwork are always self-pay and not covered by any insurance.


⚠️ Important Information About Insurance & Lab Testing


Some patients choose to use their health insurance for the laboratory portion of their Immigration Medical Exam to try to reduce their out-of-pocket cost.


However, Immigration Medical Exams are not routine preventive exams, and laboratory charges processed through insurance may be billed differently than expected. Depending on your insurance plan, you may be responsible for deductibles, copays, coinsurance, or laboratory charges that can exceed our bundled clinic pricing.


Our bundled Immigration Medical Exam pricing includes the required exam and routine laboratory tests for your age group, helping many patients avoid unexpected laboratory bills while providing a faster and simpler process.


If you would like to use your insurance for laboratory testing, we'll be happy to discuss your options before your visit so you can make the choice that's best for you.


  • The labs require blood and urine testing and you do not need to fast. One of the tests is for tuberculosis and some individuals test positive due to vaccination in other countries. If this is the case, Immigration will require a chest x-ray with a cost of $30 - $50.
  • We collect your blood and urine samples right here at the clinic and send them directly to the lab—no need for a separate appointment or long wait times.

  1. If you have vaccine records from your country, we will review them to avoid the extra cost but keep in mind that some vaccines expire, and you must perform them again even if you had them in the past.
  2. Vaccines have an extensive range in price. Our office has access to verify your information within the FloridaShot portal and obtain a full report and avoid unnecessary vaccines costs at no additional charge if vaccines have been performed in Florida.

How long does it take to complete the process for the i-693 medical exam?


  • We need to see you at the clinic at least 2 times. the initial visit to do the paperwork, labs and doctor visit and one sign and pick up the paperwork.
  • The patient can pick up the sealed envelope in 5-7 days if the labs return normal and we have all the required vaccine records or proofs.
  • Some patients require an additional chest x-ray or medical treatment if the blood or urine test come back positive delaying the process.

What do I need to bring to the i693 appointment?


You need to bring Identification, any Vaccine records from your country, and any notification or letter from immigration you received as they need to be included in the sealed envelope.

Can I send a family member to pick up my sealed envelope with form i-693?


No, the person applying and performing the exam MUST sign in person after verifying all information is correct before we seal the envelope.
