Most schools and youth sports leagues require children to complete a physical exam before they participate in any sport. Our sports physical exam can screen athletes for potential illnesses or conditions that may limit or restrict their sports activity. This exam is important because it helps to make sure kids who play sports are physically capable of meeting the demands of a sport and don’t have any conditions which can be aggravated by intense exercise.
The goal of the sports physical is to help keep sports safe and accessible to every student who wants to play!
The sports physical exam includes a medical history questionnaire, which asks about the child’s history of injury, illness or any health conditions.
Some of the health history questions are about:
- Emergency contact information
- Family history of illness
- Personal history of illness or injury
- Diagnosed medical conditions
- History of heart problems
- History of head injury
Students can obtain the Florida Certification of Immunization at Sunshine State Medical, Inc. The student must bring the records of the previous immunizations. Please call us for more information!
